Key Responsibilities\n

• Conduct individual and group therapeutic sessions tailored to meet diverse patient needs, utilizing evidence-based practices to maximize effectiveness.

• Perform thorough psychosocial assessments and mental health intake evaluations, identifying the unique challenges and needs of each patient.

• Deliver comprehensive social work services, collaborating closely with patients and their families to provide holistic support and resources.

• Work alongside other healthcare professionals to facilitate seamless patient care coordination, ensuring that necessary follow-ups and support systems are in place.

• Initiate, develop, and maintain patient treatment plans, adapting as needed based on ongoing evaluations and patient progress.

• Ensure meticulous preparation of documentation, including patient records and reports, adhering to compliance standards and best practices.

Patient Population / Client Focus\n

The primary focus of this role encompasses a diverse range of patients facing chronic health conditions, including but not limited to anxiety, depression, PTSD, and various psychosocial stressors. Our dedicated counseling approach utilizes a combination of traditional therapy and complementary health strategies, providing a well-rounded path toward recovery and improved mental health outcomes.

Qualifications\n

• A Master’s Degree in Professional Counseling from an accredited institution, showcasing a strong foundation in counseling principles.

Compensation and Benefits\n
